ADMINISTRATIVE REQUIREMENTS

1. One (1) color photograph, 4 x 4 cm (shoulders and face exposed, not headcovering, white background) recently taken (no more than six (6) Months)
2. Application form
3. Passport copy
4. Experience or supporting document for the position:
4.1 Elegibility letter issued by certification department with the approved grade and limitation ( not required for revaildation)

• Certificate of competency / seaman s book issued by pma and company letter/s stating the seagoing service not less than 36 months,
  Which may be reduced to not less than 24 months i f not less than 12 months has been served as chief mate on a seagoing ship of 500 gross tonnage or more, meeting the requirements of section a-11/2 of the stcw code.
• Assessment of competence certificate according the valid national legislation. Diploma or certificate submitted by which the coc was granted.
• Security proficiency certificate
  • Security training for seafarers with designated security duties (a-vi/6-2),
  Or
  • Ship security officer (a-vi/5)

REQUIRED VAILD COURSES

Training in maritime training centres recognized by the panama maritime authority:
• Personal survival techniques (a—vi/1 1).
• L-ire prevention and fire fighting (a—vi/1 2).
• Elementary first aid (a—vi/1 3).
• Personal safety and social responsibilities (a—vi/1 4).
• Proficiency in survival craft and rescue boats other than fast rescue boats (a—vi/2 1).
• Advanced fire fighting (a—vi/3).
• Medical care (a—vi/4—2).
• Radar, arpa, bridge teamwork and search and rescue radar navigation at management level (table a 11/2).
• Operational use of electronic chart display and information systems (ecdis) (table a—11/2).
• Use of leadership and managerial skills (table a—ii/2).
• Upgrading for masters and chief mates a— ii/2 ( not required for revalidation )

7. Valid medical certificate according the section 1/9 of the stcw 78, as amended, and/or the mlc 2006, as amended (original document).

REVALIDATION

Meet the requirments 1, 2, 3, 4, 5, and 6.

Certificate of competency & seaman's book issued by pma / diploma or certificate submitted by which the coc was granted.
Shall demonstrate continued professional competence as required in the section a-1/11, submitting at least one of the following conditions:
(CONDITION 1) Approved Seagoing Service performing functions appropriate to the certificate held, for a period of at least:


• Twelve (12) months in total during the preceding five years, Or
• Three (3) months in total during the preceding six months immediately prior to revalidating.
(CONDITION 2) Have completed satisfactorily the refresher and updating courses required by regulation 1/11 of the code and approved by PMA.
Shall meet the requirement 6.3. When it applies, according the valid national legislation..

SEAGOING SERVICE shall be on seagoing ships of 500 gross tonnage or more and stated in the seaman's book duly stamped and in company letter head. Duly letterhead detailing th e ship's particulars (imo number, call sign, type of vessel, gross tonnage, propulsion power.
